Please note the new Hepatitis A and Pnuemoccocal Conjugate Vaccine Requirements
*All vaccine doses administered less than or equal to four days before the minimum interval or age shall
be counted as valid.

VACCINE
VACCINE REQUIREMENTS UPON ENTRY INTO SCHOOL*
POLIOMYELITIS
(OPV/IPV)(OPV/IPV)(OPV/IPV)
•        3 doses of polio vaccine are required for children 3 years of age.
•        4 doses of polio vaccine are required for children 4 years of age.
•        The fourth dose is not necessary if the third dose was administered on or
after the fourth birthday.
•        If any combination of 4 doses of OPV and IPV was received before 4
years of age, no additional dose is required.
DIPTHERIA/TETANUS/PERTUSSIS
(DTP, DTaP, DT, Td)
•        4 doses of diptheria-tetanus-pertussis containing vaccine in any •        4

•        1 dose of Hib vaccine on or after 15 months or
•        Total of 4 doses of Hib vaccine with #4 given at 12 months or older, and
at least 2 months since dose #3
HEPATITIS B
•        3 doses of hepatitis B are required
VARICELLA
•        1 dose received on or after the first birthday is required.
HEPATITIS A
Children attending child-care who are two years of age or older will need two
doses of the hepatitis A vaccine separated by six to 18 months. Children must
be allowed 18 months from the date of the first dose to complete the series
before a child-care facility can deny admittance.
